Job Summary
The Supported Housing Program will deliver treatment, rehabilitative and supportive services to 130 clients “in vivo” in regular community settings (e.g., home, apartment, job site). For all Supported Housing Team clients, functioning in major life domains presents significant personal difficulties. These domains include affordable, safe housing; meaningful daily pursuits, including employment; and satisfying interpersonal relationships. Addressing the rehabilitation needs of clients in these key domains will be the Supported Housing Team’s essential purpose. The Supported Housing Team will include an on site ADMHS psychiatrist, two registered nurses, and eight staff. The foundation of the Supported Housing Team shall be integrated treatment, rehabilitation and support services.

The Rehabilitation Specialist will have responsibility for supporting each client's recovery process, helping individuals to restore competencies and gain successes in the major areas of
community living. These include: permanent, affordable housing; successful daily life
pursuits, particularly regular, competitive employment; and renewed relationships.

Minimum Qualifications
Spanish Bi-lingual/Bi-Cultural Preferred. Must have a bachelor’s degree and at least two years of supervised experience in providing substance abuse treatment interventions to persons with co-occurring psychiatric and addictions disorders. The Lompoc ACT Program is a Mental Health Services Act (MHSA)-funded, intensive Assertive Community Treatment program designed to engage with and provide services to transitional age youth (ages 16-25), adults, and older adults with serious and persistent mental illness so they can succeed in the community. Lompoc ACT provides the full range of services including engagement, assessment, individualized planning, personal service coordination, integrated co-occurring drug and alcohol services, medication support, counseling support, social skills development, supported housing, integrated vocational services, supported education and access on a 24/7, 365 day basis. Lompoc ACT promotes healing and recovery through a “whatever it takes” service delivery system.

More specifically, the Peer Specialist is a Transitional Age Youth who has received mental health or alcohol and drug recovery services, and is able to demonstrate and promote recovery from his or her personal experience as well as from the training in respected curriculums on best practices for mental health recovery. Through daily interaction with program clients, the Peer Specialist will determine the social, physical and emotional needs of clients, working directly with a diverse population of peers in a variety of settings. The Peer Specialist will assist clients in completing forms and applications; help them in the activities of daily living and provide them support as they navigate life. Using his or her unique vantage-point, the Peer Specialist will advise and accompany clients in their journey through recovery and will advocate for the client’s needs as a member of the multi-disciplinary Lompoc ACT team.

Minimum Requirements
High School Diploma and at least 1 year of employment. Must be a Transitional Age Youth who has received mental health or alcohol and drug recovery services. The Supported Housing Program will deliver treatment, rehabilitative and supportive services to 130 clients “in vivo” in regular community settings (e.g., home, apartment, job site). For all Supported Housing Team clients, functioning in major life domains presents significant personal difficulties. Transitions Mental Health Association has an opening for an Vocational Rehabilitation Specialist for the Vida Nueva Program, a large start-up program. The Vida Nueva Program is a Mental Health Services Act (MHSA)-funded, intensive Assertive Community Treatment program designed to engage with and provide services to transitional age youth (ages 16-25), adults, and older adults with serious and persistent mental illness so they can succeed in the community.
